Turn your typical grocery shopping trip into a fundraiser. On Wednesday, January 23, shop at Whole Foods Market Deerfield or Whole Foods Market Northbrook and support Be Well-Lake County, a partnership between NorthShore University HealthSystem (NorthShore) and the Lake County Health Department (LCHD) and Community Health Center. Five percent of the total sales on January 23 from these two stores will be donated to Be Well-Lake County.
Be Well-Lake County provides greater access to comprehensive diabetes care for medically underserved patients. This community health initiative provides high quality diabetes treatment, education, and support to improve diabetes outcomes in patients who normally might not have access to such resources. Be Well-Lake County offers a coordinated network of patient-centered healthcare services to patients and their families, including diabetes management for more than 670 current patients at the LCHD Community Health Center in North Chicago. Be Well also provides subspecialty care access for cardiology, ophthalmology, endocrinology, and nephrology, and a comprehensive approach to a healthy lifestyle.
Patients of Be Well-Lake County receive assistance with medical care, medication and testing supplies to help manage their diabetes. “But the unique aspects of the program are the services and support provided beyond just the basics,” explains Christina Arnold, Be Well-Lake County Diabetes Program Coordinator. “In addition to medical and even specialty care, patients can take exercise classes at a local fitness center; work with a dietician to learn how to make healthier food choices; spend time with friends and grow fresh produce in the community garden; and participate in healthy cooking demonstrations."
Funding for Be Well-Lake County is provided by NorthShore University HealthSystem and through charitable donations received from individuals.
